1. The information provided in this form will be used to assess your suitability for our hybrid coaching service. All information will be kept confidential.

2. Submitting this form does not guarantee enrollment in the coaching program. Our team will review your application and contact you to discuss next steps.

3. If accepted into the program, you agree to actively participate in the coaching sessions, whether in-person or virtual, and complete any assigned tasks or exercises.

4. Payment for the coaching service is required upfront. Refunds will be provided only in exceptional circumstances at the discretion of the coaching provider.

5. Your personal information will not be shared with any third parties without your consent, except as required by law.

6. The coaching provider reserves the right to terminate the coaching relationship if there is a breach of trust, fraud, or other extenuating circumstances.

7. By submitting this agreement, you confirm that you are of legal age and have the mental capacity to engage in this coaching program.
